– Claimant was diagnosed with Non-Hodgkin’s Lymphoma (NHL) or one of its subtypes (B-cell lymphoma, T-cell lymphoma, chronic lymphocytic leukemia, or hairy cell leukemia) no later than 10 years after last usage date
– Claimant was not diagnosed prior to the year 2000
– Claimant is currently under the age of 80
– Deceased Claimants are accepted – must be within the last 10 years
– Claimant is disqualified if any of the following apply: HIV or AIDS diagnosis, Hep B or C diagnosis. Agent Orange exposure, Organ transplant recipient, Prior chemotherapy
– Claimant has no Legal Representation
